Description

The Building 8 personnel airlock stalled mid-cycle with both interlock doors sealed, trapping two engineers for ~15 minutes until Barry on the gate manually vented and released the outer door. No injuries. Reported P1: the airlock must fail safe, and it did not.

Both doors latched, cycle hung, two lads sealed in. Barry got them out by hand. Nobody hurt, but not a nice quarter of an hour.

Watchdog hung on a stuck I/O card. Card swapped, fail-safe vent tested, now defaults OPEN on any hang. But the airlock and the cell-3 interlock share a PLC — raising DEVCON-1471 to split them. Resolved here.
